You must have 1 year of recent and relevant experience using your license. **We can assist you if you do not have all of the items below.

• A current Arizona Nursing License
• Arizona Driver’s License
• Automobile Insurance Proof
• Car registration
• Certified or original documentation (i.e. Driver’s License, Social Security Card, or Birth Certificate)
• **CPR Card (Health Care Provider)
• **TB skin test (provided by MDHH)
• **Physical – within the last year
• **Fingerprint Clearance Card
• Test – 70% is passing
• 3 work related references in the last year*

*CERTIFIED NURSE AIDE – will accept 1 personal reference and 2 professional references

Therapists [+]
